CONTRIBUTION ELECTION FORM

MOUNTAIN REGION SPEECH AND HEARING / SERTOMA CLUB 2008 GOLF TOURNAMENT

 

Major Sponsor ($5000 or more) - For contributions of $5000 or more you receive:

1) Special arrangements will be made to ensure primary visibility of your company name

a) your company will be identified as a major sponsor on advertisements for the tournament,

b) your company will be prominently featured at the golf course during the tournament, in the Tournament Program, and in the "Thank You" ad published by the Times-News.

2) In addition you will receive a complimentary 4-person team registration to play in the tournament.

Platinum Front Nine and Back Nine Sponsors ($2500 or more) - For contributions of $2500 or more you receive:

1) Special arrangements will be made to ensure primary visibility of your company name

a) at either the front or back nine of the course,

b) in advertisements,

c) at the golf course during the tournament, and

d) in the Tournament Program and in the "Thank You" ad published by the Times-News.

2) In addition you will receive a complimentary 4-person team registration to play in the tournament.

Gold Sponsors ($1000 or more) - For contributions with a total value of $1000 or more, you receive:

1) Prominent display of your company's banner at the golf course during the tournament.
2) Special acknowledgements of your company in the Tournament Program and in the Times-News "Thank You" ad.

Silver Hole Sponsor ($200 or more) - For contributions with a total value of $200 you receive:

1) Display of your company's name on a sign at a tee box for a hole during the tournament.

2) Display of your company's name in the Tournament Program and in the Times-News "Thank You" ad.

All Sponsors (any amount) - All contributors will receive:

1) Display of your company's name in the Tournament Program and in the Times-News "Thank You" ad.

 

Prize Contribution Sponsors - Non-cash merchandise and gift certificates received will be used as tournament player prizes, door prizes, or silent auction items.  Items in sets of 4 may be used as flight prizes & 130-count items can be used for golfer's goody bags.

 

Team Sponsors - You may sponsor one or more 4-person teams to play in the tournament on May 17, 2008. Fees are $80/player ($320/team).  Please indicate the name and telephone number of the team captain(s):
